Care Team Specialist (Maple St Campus)
SUMMARY
The Care Team Specialist (CTS) supports and promotes LACHC’s mission to show God’s love by providing quality, comprehensive healthcare to the homeless and underserved. The CTS ensures the safety and security of clients and staff by serving as a crisis prevention specialist at the Maple Street Campus. This individual plays a critical role in maintaining safe clinical environments and supporting compassionate, trauma-informed care for individuals experiencing homelessness.
ESSENTIAL DUTIES, RESPONSIBILITIES, AND PRIVILEGES include:
· Provide on-site safety support at the Maple Street Harm Reduction site five days per week/ as needed.
· Monitor Maple Street Harm Reduction grounds and common areas to ensure a safe and welcoming environment for staff and clients.
· Use trauma-informed de-escalation and crisis intervention techniques to support clients experiencing behavioral health or substance use crises.
· Escort clients as needed and engage in non-judgmental, supportive conversations regarding mental health, substance use, and life experiences.
· Identify and appropriately respond to behavioral disturbances, ensuring non-violent crisis intervention is used only as a last resort.
· Collaborate with clinic staff to follow established crisis response protocols and report incidents accurately.
· Maintain strict confidentiality and share client-related information only with authorized personnel.
· Performs other duties as assigned.
QUALIFICATIONS:

EDUCATION AND/OR EXPERIENCE:
· High school diploma or equivalent.
· At least 1 year of experience working with vulnerable or homeless populations in community or clinical settings.
· Ability to work outdoors and engage respectfully with individuals living in encampments.
· Proficiency in Microsoft Word, Excel, and PowerPoint.
· Demonstrated ability to manage time effectively and follow set schedules.
· Compassionate and patient demeanor with strong interpersonal communication skills.
· Knowledge of HIPAA, trauma-informed care, and harm reduction practices.
· Experience in crisis intervention or behavioral health settings.
· Bilingual in English and Spanish is a plus.

While performing the duties of this job, the employee will be exposed to carriers of contagious diseases such as TB, AIDS and other sexually transmitted diseases. The employee may have to deal with irate and irrational clients who may or may not be under the influence of mind-altering substances such as cocaine, marijuana, alcohol, etc.
